Create a risk register from a meeting.

A table plotting every risk by likelihood and consequence, with full rationale attached.

What is a risk register template?

A risk register is a structured table that records and tracks identified risks from your meeting. It serves as a central repository for documenting potential threats, and assessing their likelihood and impact. Likelihood is down one axis, and consequence level is across the other. Every risk is plotted somewhere between "rare and insignificant" and "almost certain and extreme". It's a clear way to see where the risks sit.

Contented populates the matrix from your meeting, scoring each risk for likelihood and consequence. It pulls reasoning straight from what was said in the room.

Example output

How to use a risk register

Record your meeting, choose the risk register template, and get a fully populated matrix within minutes.

1. Record your conversation

Capture your meeting in the Contented app. It works for formal risk reviews, project kick-offs, or the kind of messy strategy chat where risks come up between other topics.

2. Select the risk register template

Pick risk register from your Contented workspace. Contented identifies every risk raised, assesses the likelihood and consequence based on what was actually said, and places each one on the grid.

3. Review, export, and share

Download as a CSV, PDF or Word. Share with your team, drop it into a steering committee pack, or use it as the basis for your mitigation plan.

Who uses a risk register?

Anyone running a project, launching something new, or planning ahead. A risk matrix sounds dry until you need it, and then it's the first thing everyone asks for.

A few examples:

  • Teams implementing new tech, where safety, consent, and data reliability all need weighing up.
  • Project managers running kick-offs or steering committees where risks need to be logged formally.
  • Compliance and governance teams preparing for audits or board reporting.
  • Product launches and go-to-market planning, where commercial, technical, and operational risks all surface.

What's in a risk register template?

  • A colour-graded 5x5 matrix plotting every risk against likelihood (rare to almost certain) and consequence (insignificant to extreme).
  • A detailed write-up for each risk covering the consequence rating, likelihood rating, and the overall risk level.
  • Direct quotes from the meeting embedded in the rationale, so you can see exactly why a risk landed where it did.
  • Risks automatically sorted so the extreme and high-rated items are impossible to miss.
  • Export-ready CSV, PDF or Word formatting.

FAQs

What kinds of meetings and conversations can I record?

Any conversation, anywhere. Contented can record online, in person, even in your car. No meeting bot required. Capture client meetings, strategy sessions, board meetings, HR discussions, interviews, team catch-ups, conference debriefs, even your long solo musings on the beach.

Can I share meeting notes and documents with my team?

Yes. You can share conversations directly with individual team members or groups within your workspace. Recipients can view the conversation and generate their own documents and insights from it. You can also share with full permission, so others can edit and manage the recording. What you create can be downloaded to share as a document or CSV.

Can I record in-person meetings, not just video calls?

Yes. The Contented app lets you record any conversation directly from your phone or computer. No bots needed in the meeting. It also works offline, so you can capture meetings in locations without reliable signal and the recording will upload automatically when you're back online.
